Achieve The Best Hair Transplant Result

A Hair Transplant result evolves slowly over 12 to 18 months before full growth should be achieved; technical and medical procedures must be in place to care for the hair on removal, cleaning, slit making and placement

Graft transection is the enemy of a good result. Either via FUT or FUE transection is kept to minimum percentage and any transected grafts not placed as the probability of regrowth is minimal. Transection is when the graft is damaged, cut, dissected and can occur when being extracted or when being divided into the natural groups of follicular units

Are the grafts only being removed from a safe zone, where the hair is immune to the effects of DHT, and male pattern loss; this is easier to predict with the FUT technique compared to FUE as the zone is localised to a smaller area

Deciding the safe zone, especially with FUE is harder when hair loss is minimal as there is no obvious demarcation line and no way to say how far hair loss will progress

Miniaturised hairs in the donor area; If the % of miniaturisation in the donor is high it can mean a hair transplant is not advisable due to the trauma of extraction it will likely conclude they will not survive.

The grafts need to be cleaned, this should always be under stereo microscopes to ensure the best quality and care is taken, and then held in the appropriate holding solution until they are ready to be placed in the recipient area

Creating the recipient sites, depth control, how close the slits are made to each other, too close can impair healing, skin quality and regrowth or yield. Custom-made blades allow for greater control and higher density to be placed, safely

Follicular unit are delicate and need to be handled with care; holding the graft when being placed is important, if care is not taken too much pressure can be applied and the graft crushed, if placed it will not grow

It is very uncommon that a hair transplant performed by an experience competent doctor will not grow and the yield should be around 90-95% yield of placed grafts regrowing.

The patient also has a responsibility for graft care post operation, washing the grafted area, especially over the first 2 weeks as the scalp is healing and the grafts are bedding into the new area.

Care should be taken not to massage too hard or aggressively, not to knock your head on sharp objects; a graft can be dislodged from the recipient site and if not replaced with speed will not

As hair loss progresses hair styling can become difficult to hide the light reflection off the scalp; one reason why many shave their hair to reduce the contrast between hair and scalp. As we cannot replace like for like hair lost blocking the reflection of light becomes more important and a fundamental to achieving a natural looking and cosmetically acceptable result

Ultimately to block the light reflection and give the appearance of fullness there needs to be a minimum hair length; this allows each hair to overlap or cover the next hair more effectively. Placing the hair at a definite angle and orientation dependent on the area of the scalp, temporal recession, and top of the scalp along with the density placed and matching the hair shaft curvature will aid the illusion

It comes down to the law of physics; thick long hair will cover the scalp better than thin short hair. With thick, longer hair the light does not reflect back as easily and less scalp is visible; but we are not talking having to wear the hair very long

As hair loss progresses it is important to understand that it may not be possible to create an even density from hair line to the crown, and in some cases a lower crown density maybe required

Long term planning is important from the first procedure to ensure that the donor hair can achieve a balanced coverage; correct donor management and planning will allow even high NW candidates to achieve good coverage

As the goal of a hair transplant is to block the appearance of scalp there needs to be a balance between the density of follicular units placed and the hair characteristics that ultimately determines how long/short the hair can be. The technique used does not impact on this aspect of hair transplantation

Hair characteristics play a major role in the success of a hair transplant and how the procedure will be planned and executed; although an individual’s hair characteristics maybe similar to another’s there will be variants that determine how a hair transplant will be planned

When a hair transplant doctor assesses a hair transplant candidate they will be looking at certain attributes as to whether the donor hair area can provide a successful hair transplant result, now and over time as and when hair loss progresses

Curly or Wavy or Straight Hair

The curl of the hair shaft will alter the density required to be placed and the coverage each hair gives over the surrounding hairs. The greater the curl or wave will add to the fullness of the result, creating a better illusion of thickness and density. Different genres will genetically have more or less curl to the hair shaft, from very straight to extreme tight curls

Hair Caliber or Diameter of the hair shaft

Again, this will genetically vary dependent on ethnicity with some individuals having very coarse hair and others fine hair. Hair calibre will aid to give the coverage over the scalp, coarser hair covering a higher surface area and requiring a lower density to be placed compared to finer hair. Hair calibre can vary by as much as 30-40% from fine to thick hair and plays an important position in the planning and distribution of the grafts

Kinky or Frizzy Hair

This should not be confused with curly or wavy hair; kinky hair is the irregular pattern or surface of the hair shaft. This can give the look of increased fullness to a hair transplant result, giving more “bulk” to the hairs when grouped together. Although many people do not like to have frizzy hair it is a trait that certainly aids a hair transplant result

Contrast between Hair and Scalp Colour

As hair loss is noticeable when light shines back of the scalp, creating a contrast between the skin and hair colour the lower the contrast the less obvious hair loss becomes. Blonde hair and pale complexion or dark hair with dark skin complexion creates less contrast so gives the appearance of greater fullness of hair density. Opposed dark hair and light skin complexion will increase the contrast and require a greater density placed to reduce the contrast

The above hair characteristics are natural genetic traits we are born with; some can be manufactured to change, such as dying the hair to reduce the colour contrast and increase the illusion of fullness; but if we work with the natural hair characteristics then a perfect hair transplant candidate would have a low hair/skin colour contrast, a thick hair shaft with either curly or kinky characteristics
